What This Notice Means

The IRS needs more information about the business, estate, or trust listed on the notice. It wants to verify what was reported on Form SS-4, Application for Employer Identification Number (EIN).

What to Do

If you were or are affiliated with the entity:

  • Answer every question on the form enclosed with your notice and provide the information requested within 30 days from the date of the notice.
  • For an estate, include a copy of the deceased person's death certificate.
  • For a trust, include a copy of the Certificate of Trust, or the title and completed signature pages from the trust documents.

If you're not affiliated with the entity or you're not the responsible party:

  • Select the statement on the enclosed form showing no affiliation and give your name.
  • Return the form within 30 days from the date of the notice.

If the entity's responsible party or address has changed, file Form 8822-B, Change of Address or Responsible Party, Business.

How to Reply

The fastest way is the IRS Document Upload Tool, using the access code on your notice. You can upload photos or scans as JPG, PNG, or PDF files. You can also fax your reply to the fax number in the notice. Don't mail a reply you already faxed.

If You Don't Respond

Missing the deadline or failing to update the entity's information will delay processing of returns filed, refunds, and applying overpayments to next year's estimated tax.

After you reply, the IRS will contact you by mail if it needs more. Otherwise, nothing else is required.
