Form 668-E is a Release of Levy used in a narrow set of circumstances involving seized property. The IRS uses Form 668-E to release property when the standard release vehicle, Form 2433 (Notice of Seizure), does not fit the situation. In practice, Form 668-E tends to appear in physical seizure cases rather than bank or wage levy cases.
Why There Are Two Release Forms
The IRS maintains two separate release forms because the underlying levies work differently. Form 668-D, Release of Levy / Release of Property from Levy, releases a continuous or one-time levy on income or deposits held by a third party, including Form 668-A bank levies, Form 668-W wage levies, and Form 668-R retirement levies. Form 668-E releases seizures that were documented using something other than a standard Notice of Seizure. The practical distinction matters less than the function. Both end the levy.
What to Confirm
If you receive a Form 668-E, confirm the specific property it covers, the date the release is effective, and the signature of the IRS employee issuing it. If the release does not match the property that was seized, request a corrected form. In seizure cases where multiple items were taken, verify that each item is listed.
