What This Notice Means

The IRS needs information about one of your current or former employees. The notice goes to you as the employer.

This is an information request only. It isn't a bill and it isn't an audit of your business.

What to Do

You have two options:

  • Complete the appropriate sections of Form 15106, Request for Updated Taxpayer Information, which comes with your notice, and send it within 30 days from the date on the notice.
  • Call the number shown on the notice and give the information by phone.

Practical Tips

  • Pull the employee's personnel and payroll records before you fill out the form so your answers are accurate.
  • Answer only what the form asks. If you don't have the information, say so.
  • Keep a copy of the completed form and note the date you sent it.
  • If you're unsure the notice is legitimate, use the number printed on the notice and check it against the IRS's published contact information.

What This Is Not

This request is different from a notice of levy. A levy notice orders you to turn over wages or other money owed to the employee. An LP61 only asks for information. If you later get a levy notice for the same employee, that one comes with its own instructions and deadlines.

Keep a Record

Keep this notice with your payroll records. Call the number on the notice if you have questions.
