What This Notice Means

The IRS needs your help locating a taxpayer you may know. You received this as a third party. It doesn't mean you owe anything.

What You Can Do to Help

  • Complete Form 15106, Request for Updated Taxpayer Information, which comes with your notice.
  • Return it in the envelope provided.

If you'd rather speak with someone, call the number on your notice. Respond by mail or phone within 10 days from the date of your notice.

Give only what you know. If you don't know where the person is, say so.

Third-Party Contact List

The IRS is required by law to keep a list of third-party contacts. In some cases, it must give that list to the person your information relates to.

If you don't want to be on the list, call the IRS at the number on your notice and ask to be excluded.

Is This Real?

If you're unsure the notice is legitimate, don't use a phone number from an email or text. Use the number printed on the notice, and check it against the IRS's published contact information.

Keep a Record

Keep a copy of the completed form and note the date you mailed it or the date and time of your call. File the notice with your records.
