Use Form 15114 to state whether you agree. If you disagree, explain each item and attach relevant supporting records. If the income was already included, identify where it appears on the filed return. IRS guidance says you do not need to amend merely to report the income identified in this letter; follow its response process.

Organize the evidence

Match each disputed item to a payer statement, ledger entry, or other record. Check for amounts combined on one return line, duplicated reporting, or information belonging to another party before accepting a change.

Keep the completed response and delivery evidence. Do not confuse this verification request with a later proposed adjustment or bill.
