Letter 6534 is an outreach letter. The IRS mails it in partnership with the Centers for Medicare & Medicaid Services. It goes to people who, or whose family member, didn't have health insurance at some point last year.
The message: you may be eligible to get help paying for health insurance if you sign up through HealthCare.gov.
What This Letter Is Not
It's not a bill. It doesn't say you owe anything. It's information about coverage options.
The Financial Help Available
The Premium Tax Credit is financial assistance that may help pay the monthly cost of health insurance from HealthCare.gov. How much you may get depends on your family size and income. If you receive the credit in advance, you reconcile it when you file your tax return.
When to Sign Up
The letters go out in November. For 2025 coverage, open enrollment ran from November 1, 2024, through January 15, 2025, and for most people January 15 was the last day to sign up.
You may be able to sign up later if you have certain life changes during the year, such as losing health coverage, quitting or losing a job, moving, or getting married.
What to Do
If you already have health insurance, such as Medicare, Medicaid, or coverage from your employer, you don't need to do anything to keep it.
If you don't have coverage and want it, go to HealthCare.gov or call the Marketplace Call Center. You can also make an appointment with an assister who can help you sign up. Information on assisters is at LocalHelp.HealthCare.gov.
The IRS can't answer questions about these letters. Questions go to the Marketplace.
